• Rates for either monthly or bi-weekly pay schedules
• Life insurance premiums are “per-$5,000 of coverage”
• Recommend contacting  the AFLAC representative for current rates.
• Program and benefits are available to all District employees and are portable when the employee leaves the organization.
• No minimum participation requirements at the District level – as few as 1 participant is allowed.
• Board Supervisors are eligible to purchase benefits through this program;  as long as they are also eligible for mileage reimbursement.
